  • $225 per session

    If cost is a concern, I offer a sliding scale based on household income. I encourage you to ask about this option as your ability to comfortably access care is important to me.

    The first session is 75-minutes. Each following session is 50-minutes.
    Sessions are available in-person, online, and in your home, school or workplace. My office is located in West Los Angeles. Online video conferencing is available for clients in California and Oregon. If you are in the Los Angeles area, I can come to your home, school or workplace for an additional travel fee.

    Charges will be billed automatically to the card on file the night after your appointment.
    Credit, debit, and health savings account accepted.

    Unfortunately, due to federal law, I cannot see clients who are beneficiaries of Medicare or Medi-Cal. As a provider who is not enrolled with these systems, I am not permitted to collect payment from their beneficiaries.

    I am an out-of-network provider.
    Call your insurance to verify coverage before scheduling your first session. If you would like to submit your therapy receipts to your insurance company for reimbursement, please let me know and read below for more information.

  • travel fee - $37.50/15 minutes

    A travel fee is added if I travel to your home, school, neighborhood, or workplace. Minutes are calculated as the estimated duration to drive from my home office to your location, rounding to the nearest 15 minutes. A quote for the estimated amount will be provided when scheduling appointments. Please contact me or schedule a free consultation to see if your address is within my travel range.

    You may need to obtain permission from your school administrator or employer for my visit.

  • submitting receipts for insurance reimbursement

    Though Life Skills Occupational Therapy does not take insurance, you may be able to submit a claim to your insurance company for reimbursement of healthcare expenses. I can provide an itemized receipt and any other necessary letters or documentation at your request. There is no guarantee of reimbursement and coverage is dependent upon your insurance plan.

    Often insurance companies require a signed referral to occupational therapy from a physician for coverage. Request a referral from your physician and provide them with this referral form if needed.
